- The Queen re-appeared during the Visions in the Darkness patch in the Lair of the Ancient Queen
- At first, she was absolutely unkillable.
- She spawned with a few Olthoi Primordials
- Only with Nuhmudira's help, she could be weakened, and not killed, but chased away.
- The Queen was replaced by a 100,000hp Olthoi Sentinel after the Visions in the Darkness patch
