- Lower Corridor - Multiple portals found throughout the catacombs, see map
- Deeper Catacombs (Black Death Catacombs) - Found in the Lower Corridor
- Deeper Catacombs (Paradox) - Found in the south passage at the south end of the main room.
- Note: The Olthoi Ward Guardian must be dead for this portal to work (when killed won't respawn for six hours). See Aerbax's Prodigal Olthoi for details.

Notes
- In Heart of Woe, the BDC was updated with more difficult Olthoi and portals were added for the Lower Corridor and Deeper Catacombs (Olthoi Weapons Quest Quest).
- In Intelligent Designs, a portal to the Deeper Catacombs (Paradox) (Paradox-touched Olthoi Egg Quest) was added to the dungeon.
- In Corrupted Sovereigns, access to the Sealed Olthoi Passage (Prodigal Olthoi Queen) was made available via the Paradox Deeper Catacombs .
- In Recollections, the Black Death Catacombs Quiddity Seed was added to the dungeon.
